iPad Drawing
What am I required to do to enter to win the iPad?
Any patient of Family Chiropractic is eligible to enter a drawing for a chance to win the iPad in the following ways:
- Now through the end of 2011, each time you receive an adjustment at Family Chiropractic, you get a ticket.
- If you refer a friend or family member and they receive a visit at Family Chiropractic, you earn 5 tickets.
- In October, earn 3 tickets for registering and confirming your email address on our website www.okunchiropractic.com
- In November, earn 3 tickets for writing a review about Family Chiropractic on the web at:
www.kudzu.com
www.yellowpages.com
www.dexknows.com
www.yelp.com
- From December 1st through December 16th, bring in a new, unwrapped toy for a child and receive 3 tickets.
When will the drawing occur?
The drawing for the iPad will be held January 3, 2012. Winner does not need to be present to win.
Once the winner is selected, what are the next steps?
The winner of the iPad drawing will be required to complete a Promotion Release form within one week of notification of winning. This form acknowledges the person's eligibility wo win the prize and releases Family Chiropractic from liability once the winner takes possession of the prize. The form also makes the winner aware that the value of the prize is taxable and s/he will receive an IRS form. The iPad will be ready for pickup immediately upon notification of the win and after Family Chiropractic receives the completed release form.
Will Family Chirorpactic pay for Internet Service Provider (ISP) connectivity for the winner of the iPad?
No. Family Chiropractic will not pay for the internet service connectivity for the winner of the iPad drawing.
